Portal access, forms, what to bring, how to get test results, and how payments work — in one place.
Patient portal
We use FollowMyHealth for secure online access to your records. Log in at annistonmedicalclinic.followmyhealth.com to:
- View lab and imaging results
- Message your provider and care team
- Request prescription refills
- Manage appointments
- Update contact and insurance information
If you have not yet registered for the portal, ask at the front desk on your next visit or call 256-236-5631 to have an invitation sent to your email.
Your first visit
Please arrive 15 minutes early so our front desk can complete your registration. Bring:
- A photo ID
- Your insurance card (primary and secondary, if applicable)
- A list of your current medications, including dose and frequency
- Any relevant medical records or imaging from outside providers
Test results
Most lab and imaging results appear in the portal within 1–3 business days. If you have not heard back after five business days, please call the clinic.
Prescription refills
The fastest way to request a refill is through the patient portal. If you prefer to call, please allow 48 business hours for routine refills to be completed.
Insurance & billing
We participate with most major insurance plans in Alabama. For questions about coverage or billing, see our payment policy or call the business office at 256-236-5631. Payments can be made online at our payment page.
